Description:
-Responsible for all patient outreach and scheduling related to value-based agreements for the
practice(s). Responsibilities include but are not limited to:
- obtain all applicable and current patient rosters and contact information from payors and
internal systems.
- Outreach to applicable patients to ensure timely scheduling, or care follow-up,
necessary to support and maximize quality outcomes.
-development of and maintaining a tracking system or database that contains all relevant
or applicable patient healthcare needs to comply with and maximize value-based
arrangements, including annual wellness visits, care gaps, medication adherence,
attestations, and other needed forms or information.
- development of necessary processes to ensure timelines and information are met and
captured, as prescribed.
-ensure timely submission of all attestations, forms, or relevant materials to any, and all,
value-based contracted payors to ensure quality and revenue capture.
-serves as subject matter expert on value-based program performance for the practice(s),
including providing recommendations, and may be requested from time-to-time to
develop presentations, prepare meeting material, participate in, or lead, internal and
external meetings.
-training and education of any team members that are designated within Rise Health or
the practice(s) to support value-based tracking functions.
-identifies, notifies, and completes advance and timely follow-up with clinicians on
upcoming value-based form submission deadlines or patient appointments requiring
particular documentation.
Requirements:- Superior time management and organizational skills
- Certified Medical Assistant
- Proficient with Microsoft Office products, Google applications, and use of standard office
equipment
- Ability to work alone and be self-sufficient with minimal oversight
- Experience in patient scheduling
- Excellent verbal and written communications skills
- Coordinate and respond to patients and referring clinicians in a timely manner
- Focused on patient satisfaction
- Experience working in fast-paced environment
